Hermann, Missouri. A fireworks accident injured three workers, two seriously, operating the display for the Fourth of July celebration. Brad Link (26), James Pollard (25) and Cory Carson (17) suffered burn-related injuries from the explosion.

Shortly after the start of the display, it appeared that several of the pyrotechnic devices detonated on the ground across the river where the fireworks were being launched. A portion of the stored product for the display was accidentally ignited by burning debris from a misfire or low burst. The chain reaction explosion that followed ignited and detonated over 90% of the display product in close proximity to the company representatives.

Residents say people as far as a mile away say they felt the blast. 4.7.2005

"KSFMO and ATF jointly announce preliminary investigative findings in fatal explosion", 19.8.2005, http://www.atf.gov/press/fy05press/field/081905kc_crestline_explosion.pdf: The investigation indicates that six employees were in the process of loading aerial display fireworks from a storage magazine into a portable trailer for transportation to St. Louis, Mo. for several planned fireworks displays. The shells were "pre-squibed" and loaded into boxes. "Pre-squibed" meaned that the initiator (electric match) is attached to the fuse of the shell, a common practice in the fireworks industry. According to witness accounts, during the process of loading the boxes, one of the boxes made a sound and then exploded. The explosion of that box caused the other fireworks to ignite, causing the detonation of most of the remaining shells and the subsequent fire in both of the trailers. The exast cause of detonation of the first box is the focus of the ongoing investigation.

Calumet City, Ill. There was an estimated one hundred thousand dollars damage to the house on Yates Avenue where a teenager dropped lighted fireworks into the mail slot that ignited a fire that spread through the one-story brick bungalow. Eleanor Moll (76) was found July 3, 2004, by paramedics on her kitchen floor with no pulse and suffering from smoke inhalation. She died 11 days later in hospital without ever regaining consciousness.

Investigators ruled the fire was arson and Moll's death a homicide.

A youth (16) was charged with involuntary manslaughter in the fireworks incident that led to the death of Eleanore Moll. Cook County prosecutors decided not to charge the teen as an adult but they upgraded the charges against him. The teen was originally charged with aggravated arson. Later, the youth pleaded guilty to arson and involuntary manslaughter for placing lit firecrackers in an elderly neighbor's house, setting it on fire and resulting Eleanore Moll's death.

OSHA also cited the company for not protecting employees from hazards that were "likely to cause death or serious physical harm to employees". That carried a $4,500 fine.

This year alone an estimated 3,000 eye injuries will occur in the United States because of fireworks, with approximately 1,000 of these injuries resulting in permanent eye damage.

The most dangerous firework, and the one responsible for the greatest number of eye injuries, is the bottle rocket. A bottle rocket travels as fast as 200 miles per hour and explodes in mid-air sending glass and metal shattering into thousands of small pieces in all directions. Sparklers are also extremely dangerous. They can burn up to 1,800 degrees Fahrenheit, a temperature hot enough to melt gold.

Bonita Springs, Fla. At Lovers Key State Park, a truck packed with fireworks exploded Wednesday, July 2, 2003, killing four workers who had been unloading the shipment for an Independence Day display and injuring two others, one critically. The explosion triggered multiple secondary blasts that scattered debris around a 100-yard radius. The dead and injured are believed to have worked for Sunset Fireworks Ltd., the company that was setting up the display. The bodies of the four victims were left at the scene until midday Thursday, July 3, 2003, because authorities were concerned about the safety of the area and did not know whether all of the fireworks had detonated. A fifth victim died Thursday, July 3, 2003, at a hospital.

Kilgore, Texas. An explosion Thursday evening, July 3, 2003, destroyed about half the large metal warehouse of Lamb Entertainment near the downtown area. 10 or 15 smaller explosions followed the initial explosion. Thousands of pounds of fireworks inside the warehouse and packed in the tractor-trailer rig exploded. The blast shot fireworks and other debris for several blocks and damaged or destroyed six homes east of the warehouse and displaced about 25 people. Four nearby businesses were damaged. At least five people were injured. The Kilgore Police Department evacuated an area around the explosion for several blocks. Three people were missed since the explosion. Search teams working overnight found the bodies of Marvin Lamb (70), father of the warehouse owner and of two workers, Melinda Edmundson (25) and Martin Donner (58) early Friday, July 4, 2003; they were inside the building when the first blast occurred.

Kilgore public safety director Ronnie Moore has said a spark, perhaps from static electricity, probably caused the explosion.

1992-2001 there have been 557 major fire and explosion incidents involving illegal fireworks reported to the Massachusetts Fire Incident Reporting System (MFIRS). These 558 fires and explosions caused 20 civilian injuries, 7 fire service injuries, 3 civilian fatalities and an estimated dollar loss of $762,525. This is quite a substantial amount since most fireworks related fires are outside brush fires and do not usually have high loss figures. A majority of these fires occurred during the week of the Fourth of July holiday.

Syracuse. Two families are suing the town for injuries their children suffered during a July 4 fireworks display at a town park. Mike and Janet Cavender’s 5-year-old son, Stephen, suffered first degree burns to his neck when a ground display exploded unexpectedly and shot into the crowd gathered to watch the fireworks. Christopher Sr. and Vickie Wooldridge's son, Christopher Jr., 2, suffered a small cut beneath his eye, minor burns and a possible ruptured eardrum during the incident. The Cavenders and the Wooldridges are claiming that the town "was negligent in its capacity as the authority to maintain the Syracuse beach and park," that it "breached its duty to provide a safe environment for the patrons visiting" and failed "to maintain a safe distance between the fireworks launch site and the spectators." 2.12.1997 http://www.timeswrsw.com/archive/1997/N1202973.HTM

South Jersey. A state appeals court set aside a $1.6 million jury award to a Woodbury Heights boy who lost most of his left hand in a 1995 fireworks explosion.

The court reinstated a $977,300 damage settlement Brenden Wasniewski's family reached in 2001 with the fireworks manufacturer and its insurance carrier. The Appellate Court decision found Deptford Township was not liable for the undetonated device Wasniewski – then 10 years old – found after a Deptford Day fireworks show at Fasola Park. Wasniewski took the device home and ignited it about a month later. The explosion left him with only his little finger and part of his thumb.

The case went to trial in 2001: a jury awarded $1.6 million to Wasniewski, including $896,000 to be paid by Deptford. But the appeals panel, noting that the legal issue appears to be unprecedented in New Jersey, reasoned that the rocket the boy took home was not "public property (judges: the firework once removed from the park was no longer 'controlled by the public entity')." 3.6.2003

The Insurance Company must pay $896,000 to Brenden Wasniewski, who was 10 when he detonated an unexploded fireworks shell he found in a township park. The Insurance Company provided insurance for the fireworks company, which had staged a fireworks display during a township event at the park.

Serious Eye Injuries Associated with Fireworks -- United States, 1990-1994
MMWR 44(24);449-452 , Publication date: 06/23/1995

Eye injuries caused by fireworks are often severe and can cause permanently reduced visual acuity or blindness. Findings from the National Electronic Injury Surveillance System database maintained by the U.S. Consumer Product Safety Commission (CPSC) indicate that approximately 12,000 persons are treated each year in U.S. emergency departments because of fireworks-related injuries; of these, an estimated 20% are eye injuries. To improve characterization of fireworks-related eye injuries, data were analyzed from the United States Eye Injury Registry (USEIR) for July 1990-December 1994 and from the Eye Injury Registry of Alabama (EIRA) for August 1982-July 1989. This report summarizes the findings of these analyses.

Hallett, Oklahoma, Pawnee County. A series of explosions leveled the Aerlex Corp. fireworks factory, killing 21 people. Among the dead was Harold Dean Harper (18), stepson of Aerlex owner Alan Johnson, who was injured in the blast.

The cause of the blast was ruled accidental. It occurred as Harper and a coworker were unloading cans of marble-size explosives from a pickup. It is believed that the two workers allowed the metal cans to scrape along the bottom of the truck, causing enough friction to ignite some loose flash power in the truck bed. The explosion at the pickup may have caused a chain reaction, destroying the rest of the factory – a large steel building and seven smaller ones. Bodies were blown as far as 200 yards from the factory. 25.6.1985

Newport. 2.4.1981, Blast at Johns Street at 12:17 p.m.: Steve Thomas (26) and Billy Chip West (21), two roofers, died. 34 of the injured people were treated in hospital, but none suffered life-threatening injuries.

Suspicion soon turned to illegal fireworks as M-80s were found littering the ground. While some police officers concentrated on the blast scene, other officers fanned out infor other illegal fireworks manufacturing places and storage buildings. Police said they believed the Newport building was being used as a factory for fireworks.

The Red Cross provided temporary shelter for the displaced.

By April 4, the damage estimate topped $2.4 million with a total of 260 buildings suffering some amount of damage. Of those, 13 were listed as destroyed and 37 as structurally damaged. 45 cars and trucks were damaged.

A central figure in the investigation was Victor Scharstein. In October 1982, Scharstein was found guilty of operating an illegal fireworks operation that killed the two men. He was sentenced to 20 years in prison. He was released in 1988 after serving six years in jail.

Chicago. A match carelessly dropped by one of the workmen in the fireworks factory of M. SHURE, at West Van Buren and Halstead Streets, caused a terrible explosion. A number of the employes were badly hurt, and the flying rockets and candles struck several people who were passing, a man fully 400 feet distant by a rocket, which hit him squarely in the back. The explosion demolished nearly all the windows in the block and the SHURE building, which is a four-story structure, was almost entirely destroyed. Immediately following the explosion the building was ablaze from cellar to roof. The loss is estimated $60,000. The explosion caused a panic in that portion of the town. 8.6.1897
